Early learning provider must receive for each enrolled child: A current and complete department of health CIS A current COE certificate Accept children without current immunizations, and early learning provider must give written notice to parent/guardian stating the child maybe acceped if: parent/guardian provides written proof scheduled appointment to be immunized parent/guardian provides signed and dated statement detailing when child's immunizations will be brought up to date.
